Statements can get lost in the mail. Other reasons include insureds moving or providing incorrect address information.
Even though a policy premium may have been paid in full, subsequent endorsements or an uprate can increase the premium mid-term. If you investigate, you may find that an endorsement was made to the policy, and as a result, a new statement was sent to an insured with a balance due.
